Request for access to the cemetery for work

Documentation to be submitted by the company to carry out work at the cemetery:

1. Request for access for work at the Cemetery art. 69;
2. Original/Certified Copy of the Certificate of Registration with the Chamber of Commerce/Self-certified declaration signed in front of an employee of the Cemetery office;
3. Photocopy of the company owner's document;
4. Insurance Policy, valid for ___ years starting from ______ next expiry ______, for civil liability, for the insured amount of (€ 258228,45 - minimum 500.000.000 lire);
5. Payment of €100.00 as an administrative authorization, valid for one year, for the execution of works inside the cemetery, to postal account no. 13783980 in the name of the municipality of Barcellona PG - Treasury service;
6. DURC;
7. DVR;
8. Declaration in lieu of a sworn statement, art. 14 Legislative Decree 81/88
